Enrollment Form

Please complete the this online enrollment form before purchasing course materials. Upon submitting your form you will be directed to the online store where you may select your course date and course materials. If you prefer to download and print the form please click Form print, and fill out. Fax the completed form to 530-347-4285. All student enrollments must be paid two weeks in advance of scheduled class. Class materials will be mailed to the address provided on this enrollment form with class location information. Course materials are mailed to students two weeks prior to class. No materials will be mailed inside this two week window prior to scheduled class date.

Payment Method:

  • PayPal: (preferred Method) Use PayPal’s secured payment system to purchase course materials. It is quicker and secure. It is not necessary to have a PayPal account to make payments by credit card or debit cards.
  • Corporate Check: Corporate Checks must be received three weeks prior to start of scheduled class to allow processing of enrollment. Make corporate checks payable to Swim Pool Safe™. Mail completed enrollment form and payment to: Swim Pool Safe, P.O. Box 1534, Cottonwood, CA 96022
