Enrollment Form Please complete the this online enrollment form before purchasing course materials. Upon submitting your form you will be directed to the online store where you may select your course date and course materials. If you prefer to download and print the form please click Form print, and fill out. Fax the completed form to 530-347-4285. All student enrollments must be paid two weeks in advance of scheduled class. Class materials will be mailed to the address provided on this enrollment form with class location information. Course materials are mailed to students two weeks prior to class. No materials will be mailed inside this two week window prior to scheduled class date.
